How do I join the practice?

If you live within our catchment area and wish to register at this practice please ask the receptionist for details about registering.

The registration forms will ask you for details such as your name, address, date of birth, telephone number and previous address and doctor. These details are needed to help obtain your medical records from your previous doctor and the form needs your signature. You will also be given forms to fill out which cover your past medical history and various lifestyle questions.

After completion of these forms you may be offered a New Patient Check. If you have complicated medical problems the nurse may refer you to the doctor for review.

If the practice feels they are unable to accept you as a patient it will explain the reasons for this to you. The practice can decline to register patients for various reasons; these include living outside the catchment area.

Please be aware that we do not accept forms sent over the internet.
